Social & Community participation
Assisting participants to participate actively in community, social and civic activities. Development and training to increase your skills so you can participate in community, social and recreational activities.
in Home Support
This involves house maintenance and cleaning participants house. The participant will be involved in all the moments of the service and will get the opportunity and be encouraged as well as shown how to do different household tasks. The service will be provided by a professional cleaner if needed and support worker.
Transport
Focuses on transporting clients to key places they need to go to. Vehicle Insurance is required in the case that the support workers are using their personal vehicles and the company is providing comprehensive car insurance for all the company vans and vehicles. The company will offer the member a chance to have the ride with the support worker if they are both able to ride, considering with the thinking of the environmental policy of Hope Care.
Support Coordination
Dealing with participants and how they transition within the NDIS. Hope Care will not provide support coordination to own participants as this is seen as conflict of interest which is aiming to the service quality assurance.
Allied Health
Therapeutic supports are provided to assist participants to apply their functional skills to improve participation and independence in daily, practical activities in areas such as language and communication, personal care, mobility and movement, interpersonal interactions and community living. Home Modification
Incorporates design and subsequent changes to the participant’s home. Home modification design and construction includes installation of equipment or changes to building structures, fixture or fittings to enable participants to live as independently as possible or to live safely at home.
